"Special Appearance By" usually seems to be given to actors who are wellknown outside of the film but are basically just a glorified cameo or small secondary character IN the film. Sometimes it's been applied to characters who are actually somewhat large (Halloween: Resurrection gave a special appearance credit to the guy who played Rudy, I'm pretty sure - very strange).


"And..." has a bigger range. If it's an ensemble, they could be as large and long-lasting in the film as the other leads. If there's only one or two big leads, they could be dead by the half hour mark. It could also just be an alphabetical order sort of thing depending on the layout of the poster/credits. It's a lot harder to figure out what "And so-and-so" means than it is to figure out what "Special Appearance By..." means for a character/actor. :P
