I'm asking more about books that aren't exactly scary in the 'horror' sense, but material that simply makes you sick, disturbed, and that you struggle to even get through. I've never been too disturbed by a book, but Palahniuk's 'Haunted' is definitely a wild ride! In terms of a psychologically disturbing book, 'Thinner' by Stephen King got to me, purely as I was suffering from an eating disorder at the time of reading.
