 I can understand that, but at least his personality seemed to be pretty different to me. Less naive, more calculated and less 'good'.

Yes he has... what you see on that keychain and in his artwork is his real face. He has no prominent nose (it's flattened) or ears because he cut them off, and had his jawline, hairline, nose and cheekbones shaved down to better disguise himself. Those are his wounds. I've never heard of Decoy being burned, though. In backstory files, the Database or in MGS1 dialogue, anywhere.
