I shall get to the point and if this has been discussed, I am sorry, however I hope the picture is pretty enough. When I watched the VGA2012 Phantom Pain Trailer, GDC 2013 metal Gear Solid v: the Phanton Pain trailer and gameplay video GDC 2013, something annoyed me. It was the Horn. The Horn at the end of the MGSV: The Phantom Pain trailer can be seen in the gameplay video and the VGA2012 trailer. At the start of the trailer and when Big Boss enters a coma there is no Horn. However, when he wakes up from his coma the horn shape can be seen underneath the head bandage. What they hell did they do to him when he was in his coma, other than losing his arm and maybe have his DNA stolen.

Some times when something is lodged in the head a certain way, it's lethal if it is removed, so it is kept in.
