How does he manage to get that thing stuck in his forehead if he was in a coma? One would presume it got lodged while Mother Base was under siege, but again, in the hospital scene, no horn.

Personally I think he looks ''healthy'' compared to the skinnier BB in the hospital. The answer? Who knows WHEN he is on the bike in terms of story. I'm a bit surprised that people are confounded by this. Remember the car with the medicine crashing? Might of got the debris from that? Who knows, but there are multiple possibilities I'm sure.
