Personally I think he looks ''healthy'' compared to the skinnier BB in the hospital. The answer? Who knows WHEN he is on the bike in terms of story. I'm a bit surprised that people are confounded by this. Remember the car with the medicine crashing? Might of got the debris from that? Who knows, but there are multiple possibilities I'm sure. The thing is, he also has a cybernetic arm in the bike scene, so that's why I think it's after the hospital scene.

Ishmael said hes been watching him for over 9 years, maybe his is BB"S bodyguard, why'll hes been in the coma. But he did not do such a good job of protecting him. BB got caught in an explosion of some kind and took shrapnel to the head his arm could have been damage too. the map in the background of the hospital room looks like Cyprus. BB's could have gotten hurt in Turkish invasion of Cyprus will he was in his coma. Wikipedia Landing of Turkish troops in the northern part of Cyprus on July 20, 1974. Fall of the Greek military junta in Athens three days later, on July 23, 1974.
