Official MGS V Thread [Possible Spoilers & Nudes Of Zuben Inside]; PS3/Xbox 360/PS4/XB1

Maybe they've parted ways before Ocelot appears but I don't think that's it. As some already said, I believe it's some kind of subconscious shit guiding BB through the hospital incident. He physically moves BB though, so I don't think he's just his imagination... Just because he appears to physically touch and interact with BB, doesn't mean he really is. It could be a visual representation of BB standing up on his own, sloppily mind you, but being so out of it, he is imagining someone is there helping him. When Ishmael rolls him over, it could just be BB rolling over on his own, easily done by yourself. Just because we SEE Ishmael do this stuff, doesn't mean he really is. This stuff has happened in movies, too, during dream-like states or hallucinations.

Ishmael hands him a syringe (which could just be another subconscious thing but with a placebo kind of effect just to stimulate BB's reaction to the situation), when he's crawling on the floor Ishmael doesn't do shit and, eventually, BB gets up by himself. All the other stuff could have been done by himself too as Scotia said.

Otherwise we'd have another RE5-IT-WAS-JILL-moment, so it's possible BB realizes the man isn't real early in the game, but still keeps seeing him and listens to his advice rather than try to shoo him away.

True.... tbh I'm not really sure what I want it all to be and mean, I would really love BB to have serious mental problems during MGSV and that opening shown at GDC was fucking awesome... but with fire whales etc I guess it would be cool if Ishmael wasn't a real person? I don't know lol... though I have a feeling that alont of these visions and such will be explained via youth-mantis
